Abstract

Formulations for oral transmucosal drug delivery (OTDD) have been available since the 1980s. However, the number of drugs currently administered via the buccal and sublingual routes remains quite low. Currently, there is also limited development of candidates or new chemical entities for dosing via these routes. Considering that OTDD is associated with greater and faster systemic absorption compared with conventional oral drug delivery, this drug delivery route represents an area with considerable potential for development by pharmaceutical scientists. Clearly, OTDD will also be attractive for specific patient groups with difficulty in swallowing, gastric problems, or limited access to water, including children and the elderly. This review examines a number of aspects related to OTDD including the anatomy of the oral cavity, models currently used to study OTDD, as well as commercially available formulations and emerging technologies. Key drug properties that influence permeation of molecules in the oral cavity are discussed in detail. The challenges facing the formulator in progressing OTDD medicines including drug properties and the physiology of the oral mucosa are also considered. The limitations of current methodologies to study OTDD are considered as well as recent publications and new approaches which have advanced our understanding of this route of drug delivery.
