Abstract

Paranoid personality disorder (PPD) affects 2.3% to 4.5% of the population, with 10% to 12.6% of the general population partially affected by some of its symptoms. It is a Cluster A personality disorder characterized by pervasive, strongly-held, unfounded mistrust or suspiciousness of others (e.g., extreme persecutory thoughts that others are out to harm them). PPD exists on a continuum of severity in the clinical and community populations. The causes of PPD are contests as there are overlapping environmental and genetic risks with schizoid and schizotypal personality disorder. While the majority of individuals develop these traits in adolescence and young adulthood, more recent evidence suggests that a small group of young children also self-report high levels of suspiciousness. Much progress has been made on the causes and treatments of PPD and these remain significant areas of research and clinical interest. Understanding PPD and its attenuated forms is crucial to understanding the etiology of schizophrenia-spectrum disorders
