Abstract

After a curative resection of stage III colon cancer adjuvant chemotherapy improves overall survival and decreases recurrence rates. In the early 90's the combination of 12 months 5-fluorouracil/levamisole was recommended as standard therapy. Due to the results of large studies treatment recommendations were changed to a combination of 5-fluorouracil/leucovorin (Mayo-Clinic- or Roswell-Park-regimen). Today it remains still unclear whether patients with stage II colon cancer benefit from adjuvant chemotherapy. Development of new therapeutic agents like irinotecan or oxaliplatin in combination with 5-fluorouracil/leucovorin may improve efficacy. First data of a large trial with high-dose 5-fluorouracil/leucovorin + oxaliplatin are published. It was demonstrated that 5-fluorouracil/leucovorin plus oxaliplatin led to a significant reduction in the risk of relapse compared to the 5-fluorouracil/leucovorin combination. Today, data are missing whether the oral 5-FU-prodrug, capecitabine, will be equivalent to the standard regimens or even better in combination with oxaliplatin.
